Forms and Guidance

The application forms which need to be completed depend on the nature of the intended investment, ownership and governance model. For straightforward applications we will provide you with all the forms after the introductory phone call.

Where there are more complex business arrangements we may phase the issue of the forms to help applicants manage the process. We will be able to determine the nature and type of application forms to be completed by the owners and managers after the licensing meeting.

The CLC will carry out a Disclosure and Barring Service (DBS) Standard Check on Booms and Registered Managers.  The CLC will carry out these checks with an approved Credit Agency.

Practice Licence applicants must demonstrate in their replies to the CLC Licences Outcomes that they understand how to identify and manage the risks posed to the business and can operate in compliance with the CLC regulatory expectations. Applicants are strongly recommended to familiarise themselves with the CLC’s Risk Agenda publications which details issues that CLC staff have encountered during their monitoring and inspection work.
